It is important to stay on top of address updates for all insurance companies
This determines where your EOBs, ERAs and checks will be sent.
To update an address for Anthem Blue Cross:
1. Fill out and sign the attached form
2. Fill out and sign an updated fw9 (linked here)
3. Send both signed documents via email to ProviderDatabaseAnthem@anthem.com
4. Send a copy to providerrelations@helptherapy.com so we can keep your information up to date!
