EOB stands for Explanation of Benefits. This document is sent to along with your check (or alongside your direct deposit if you have enrolled in EFT or Electronic Funds Transfer).
We must receive a copy of your EOBs in order to reconcile your account.
EOBs should be sent into our billing department 1x per week via email to billing@helptherapy.com or via fax to 858-244-0990.
